  • Malignant tumor of primitive neural crest cells
  • Most common extracranial solid malignancy in children
  • Third most common pediatric malignancy
  • Commonly arises from adrenal gland but can arise anywhere along sympathetic chain
  • Surrounds and engulfs, rather than displaces vascular structures
  • MIBG uptake related to catecholamine production (70%)
  • Osseous metastasis with predilection for cranial sutures
  • Peak age at presentation is 22 months (versus 3.6 years for Wilms tumor)
  • 70% have metastasis at presentation
  • Often has calcifications

Wilms tumor - slightly older presentation, claw sign, displaces vessels
