Langerhans cell histiocytosis

aka Langerhans cell histiocytosis (LCH), histiocytosis X, eosinophilic granuloma


Chest

  • Reticulonodular opacities in upper and middle lung zones
  • Bizzare cysts with thick or thin walls
  • Normal intervening lung
  • Diffuse destructive disorder of distal airways caused by granulomas containing Langerhans cells
  • Probably an allergic reaction to smoke


Musculoskeletal

  • Proliferation fo histiocytes
  • Varies in presentation but only in young (<30) patients
  • Vertebra plana
  • Geographic lytic lesion without sclerotic boarder, no matrix
  • Skull lesions with beveled edge (uneven osteolysys of inner and outer tables)
  • Localized versus systemic disease has varying prognosis
