- Carbon Monoxide - bilateral globus pallidus T2 hyperintensity, hypodense on CT, restricted diffusion
- Wilson's disease - variable signal in basal ganglia due to copper accumulation, classically putamen, giant panda face
- Methanol - bilateral putamen necrosis with high T2 signal
- CJD - high T2 signal in caudate and putamen
- Leigh's disease
