- Acute - mimics pulmonary edema
- Chronic - revese of edema with peripheral consolidation, photographic negative of edema
- BAL demonstrates eosinophilia, but no peripheral eosinophillia
- Rapid response to steroids
- Patients present with dyspnea, fever, chills, night sweats, weight loss
- Can mimic BOOP/COP
