- Bicuspid valve is the dominant cause below age 70 and calcific degenerative above 70
- Post stenotic dilatation of ascending aorta
- Left ventricular hypertrophy
- Can perform Agatson score of valve calcium with score of 1100 corresponding to severe AS
Grading
- Mild: Aortic valve orifice area (AVA) > 1.5 cm²; mean pressure gradient < 25 mmHg
- Moderate: AVA 1-1.5 cm²; transvalvular pressure gradient 25-40 mmHg
- Severe: AVA < 1 cm²; transvalvular pressure gradient > 40 mmHg
- Critical (surgery indicated): AVA < 0.7 cm²
