  • Bicuspid valve is the dominant cause below age 70 and calcific degenerative above 70
  • Post stenotic dilatation of ascending aorta
  • Left ventricular hypertrophy
  • Can perform Agatson score of valve calcium with score of 1100 corresponding to severe AS


Grading

  • Mild: Aortic valve orifice area (AVA) > 1.5 cm²; mean pressure gradient < 25 mmHg
  • Moderate: AVA 1-1.5 cm²; transvalvular pressure gradient 25-40 mmHg
  • Severe: AVA < 1 cm²; transvalvular pressure gradient > 40 mmHg
  • Critical (surgery indicated): AVA < 0.7 cm²
